چکیده

Reconstruction of small dural defects is independent of which technique is used, and vascularized tissue does not appear to be critical. In contrast, large dural defects have been traditionally repaired using regional vascularized flaps, as they promote healing and diminish complications. Recently, novel regional vascularized flaps that do not require cutaneous incisions have been designed: nasoseptal (NS) flap, posterior pedicled inferior turbinate (IT) flap, and pedicled palatal flap. The development of the NS flap has partially overcome the challenge of obtaining vascularized tissue to cover large defects after EEA. However, these flaps cannot be used in patients who have undergone previous posterior septectomies, wide sphenoidotomies, or have tumor involvement of the septum. In an attempt to use new sources of vascular supply and to keep pace with rapidly expanding reconstructive requirements, we investigated the anatomical foundations for novel modifications of the occipital galeopericranial (OGP) flap for reconstruction of skull base defects. Use of a galeopericranial flap for the reconstruction of anterior cranial base defects.

BACKGROUND To evaluate the efficacy of using a galeopericranial flap for reconstruction of anterior cranial base defects. METHODS In Linkou Chang Gung Memorial Hospital from February 1994 to November 2003, 25 patients who had tumors of the skull base underwent craniofacial resection, and a galeopericranial flap was used to reconstruct the anterior cranial base defect. The galeopericranial fla...

Pedicled Facial Buccinator (FAB) flap: a new flap for reconstruction of skull base defects.

BACKGROUND The expansion of endoscopic endonasal skull base surgery has resulted in an increased demand for reconstructive options. Reconstruction with vascularized tissue has proven indispensable for reliably separating the cranial contents from the paranasal sinuses following extended endoscopic endonasal approaches (EEA). The introduction of the Hadad-Bassagasteguy flap (vascular pedicle nas...

Facial artery musculomucosal flap for reconstruction of skull base defects: a cadaveric study.

OBJECTIVES/HYPOTHESIS Failure in skull base defects reconstruction following tumor resection can have serious consequences such as ascending meningitis and pneumocephaly. The nasoseptal flap showed a very low incidence of cerebrospinal fluid leak but is not always available. The superiorly pedicled facial artery musculomucosal (FAMM) flap has been successfully used for reconstruction of head an...

Palatal flap modifications allow pedicled reconstruction of the skull base.

OBJECTIVES Defects after endoscopic expanded endonasal approaches (EEA) to the skull base, have exposed limitations of traditional reconstructive techniques. The ability to adequately reconstruct these defects has lagged behind the ability to approach/resect lesions at the skull base. The posteriorly pedicled nasoseptal flap is our primary reconstructive option; however, prior surgery or tumors...

Endonasal endoscopic skull base multilayer reconstruction surgery with nasal pedicled mucosal flap to manage high flow CSF leakage.

AIM Multilayer reconstruction of skull base using nasal pedicled mucosal flap has been widely accepted as a standard method for repairing high flow cerebrospinal fluid [CSF] leakage. In this study, we analyzed our outcome and summarized several valuable operation experiences from this technique. MATERIAL AND METHODS This study included 20 consecutive patients who underwent endoscopic endonasa...
